Agency vs in-house medical sales hiring
Recruiting agencies and in-house teams both place device reps—but the economics, control, and best-fit roles differ. Use this comparison before you write another contingency agreement or another vague LinkedIn post.
Side-by-side comparison
Factual trade-offs for US medical device and medtech employers—not a claim that one model always wins.
| Factor | Recruiting agency | In-house + specialty platform |
|---|---|---|
| Cost model | Contingency ~25–35% of first-year OTE, or retained fees on senior seats | Platform/subscription + hiring manager time; first specialty post often free to start |
| Control | Recruiter owns outreach narrative, timing, and often first screens | You own JD, specialty filters, scorecards, and interview cadence |
| Speed to first slate | 2–6 weeks common for a presented shortlist | Inbound within days when the post is specialty-clear and in the right channel |
| Best fit roles | Confidential VP+, scarce therapy experts, first commercial hire at some startups | Territory reps, clinical specialists, associate reps, repeat annual hiring |
| Risk if JD is vague | You still pay when a hire closes—even if the search burned weeks on wrong profiles | You see mismatch fast and can rewrite the post before fees compound |
Choose agency when
- Confidential leadership or VP Commercial search
- Truly scarce therapy-area experts after a failed internal run
- You need an external network you do not have in-house
- Board/investor pressure requires a named search partner
Choose in-house when
- Standard open-territory or specialty rep seats
- You hire 2+ device reps per year
- You want specialty matching before the first phone screen
- You already have a hiring manager who can run a tight process
Hybrid is common
Many commercial orgs keep agencies for scarce leadership seats and run specialty self-serve for territory volume.
- Post the territory seat on a specialty board while an agency runs parallel outreach on scarce profiles
- Keep retainer/contingency for RSD+; self-serve mid-level territory fills
- Use the same phone-screen scorecard whether candidates come from agency or inbound
- Measure time-to-slate and specialty hit rate—not just “apps received”
Avoid this false choice
“Agency or nothing” ignores specialty job boards. “In-house or nothing” ignores that retained search still wins for confidential VP work. Match the model to the seat—not to habit.
